Please review the below in preparation for your upcoming first appointment. We look forward to meeting with you.

COVID-19 PRECAUTIONS: Please visit our website at https://www.360nmt.com/covid19-info to read our current policies regarding COVID-19.

Forms:  Your complete health history is required in order for us to treat you safely and effectively. You have three options for completing the form:

  1. We will email you a link ahead of your appointment to fill it out securely and privately through our HIPAA compliant scheduling program.

  2. You can also download and fill it out prior to your first appointment and bring it with you. Download here and, if possible, fax (781-209-7301) or email back to info@360nmt.com in advance of your initial appointment. 

  3. It’s also possible to come in 15 minutes early to fill out a paper version, but please let us know if that is your preferred method.

Bring with you: A face-covering is required for the duration of your visit. Please also bring a pair of shorts or loose clothing for postural assessment at your first visit.  

When you arrive: Please wait outside in your car until the time of your appointment. We will NOT be notifying you when it is time to come in. If your therapist is not at the front desk, please have a seat and they will be out to greet you shortly.

Directions/Parking: We are located at 1077 Lexington Street, Suite 202, Waltham, MA 02452. Take exit 28A Trapelo Rd toward Belmont. Turn left onto Lexington Street and our building will be on the right. Subway and Supercuts are on the first floor. Walk to the left side of the building for our entrance. Our building has plenty of free parking.  

We are the 1st door on the left on the 2nd floor.  

Payment/Insurance: We accept Mastercard/Visa/Discover and personal checks for payment. A credit card is required on file to secure your appointment. We do not direct bill health insurance. Massage treatments for pain management & injuries are eligible for FSA/HSA coverage, and many patients pay with their benefit cards if they have pre-tax health spending accounts.  

Cancelation Policy: In the event that you need to cancel an appointment, we appreciate your consideration of our 24-hour cancelation policy. Any cancelation with less than 24 hours’ notice will be responsible for the full payment. With short notice it is difficult to fill an open appointment, and we have many patients on our waitlist. Late cancelations due to emergencies or sudden illness may not be charged. If patients arrive for their appointments and are ill, 360 NeuroMuscular Therapy has the right to refuse treatment and charge the full price of the appointment. Thank you for your understanding & consideration.  

Our Online Scheduling System: Some appointments may be managed through our online scheduling system (called “Jane”) on any browser. You can access our online system through our website (See “Appointments” page). When booking online, you must pay for your appointment to reserve your time slot. Appointments are not booked unless you receive an email confirmation.
